{"id":232,"date":"2025-06-20T10:09:42","date_gmt":"2025-06-20T10:09:42","guid":{"rendered":"https:\/\/www.paedsurgeryindelhi.com\/blog\/?p=232"},"modified":"2025-06-20T10:26:19","modified_gmt":"2025-06-20T10:26:19","slug":"posterior-urethral-valve-puv-in-children-understanding-and-managing-a-common-cause-of-urinary-obstruction-in-boys","status":"publish","type":"post","link":"https:\/\/www.paedsurgeryindelhi.com\/blog\/posterior-urethral-valve-puv-in-children-understanding-and-managing-a-common-cause-of-urinary-obstruction-in-boys\/","title":{"rendered":"Posterior Urethral Valve (PUV) in Children: Understanding and Managing a Common Cause of Urinary Obstruction in Boys"},"content":{"rendered":"\n<h2>How Does PUV Present?<\/h2>\n\n<p>PUV can be detected at different stages:<\/p>\n\n<ul class=\"list01\">\n  <li><strong>Before birth:<\/strong> Detected during a routine antenatal ultrasound showing a distended bladder, bilateral hydronephrosis (swelling of kidneys), or if the kidneys are dysplastic in severe cases then &#8211; oligohydramnios (reduced amniotic fluid).<\/li>\n  <li><strong>In newborns:<\/strong> Poor urine stream, urinary retention, abdominal distension, or urinary tract infection (UTI).<\/li>\n  <li><strong>In infants or older children:<\/strong> Repeated UTIs, poor growth, weak urinary stream, or symptoms related to chronic kidney disease.<\/li>\n<\/ul>\n\n<h2>Diagnosis<\/h2>\n\n<p>If PUV is suspected, the following investigations help confirm the diagnosis:<\/p>\n\n<ul class=\"list01\">\n  <li><strong>Ultrasound of kidneys, ureters, and bladder (KUB):<\/strong> Shows thickened bladder wall, Dilated Posterior Urethra \u2013 Key hole Sign, dilated ureters, and hydronephrosis.<\/li>\n  <li><strong>MCU (Micturating Cystourethrogram):<\/strong> A contrast study of the bladder and urethra, this is the definitive test showing the dilated posterior urethra and irregular shaped bladder with or without VUR.<\/li>\n  <li><strong>Blood tests:<\/strong> To assess kidney function (urea, creatinine).<\/li>\n<\/ul>\n\n<p><img decoding=\"async\" src=\"https:\/\/www.paedsurgeryindelhi.com\/blog\/wp-content\/uploads\/2025\/06\/puv1.jpg\" \/><\/p>\n\n<h2>Management and Treatment<\/h2>\n\n<p>Management depends on the severity of obstruction and kidney function:<\/p>\n\n<h4>Initial Stabilization<\/h4>\n\n<ul class=\"list01\">\n  <li><strong>Catheterization:<\/strong> To relieve bladder pressure temporarily and assess urine output.<\/li>\n  <li><strong>Antibiotics:<\/strong> For treatment or prevention of infection.<\/li>\n  <li><strong>Monitoring kidney function and hydration status.<\/strong><\/li>\n<\/ul>\n\n<h4>Definitive Treatment: Endoscopic Valve Ablation<\/h4>\n\n<ul class=\"list01\">\n  <li>This is the gold standard treatment.<\/li>\n  <li>A pediatric urologist or surgeon uses a small cystoscope to visualize the valve and cut it using electrocautery or a cold knife.<\/li>\n  <li>This relieves the obstruction and allows normal urine flow.<\/li>\n<\/ul>\n\n<h4>In severe cases<\/h4>\n\n<ul class=\"list01\">\n  <li>If the renal functions are deteriorating<\/li>\n  <li>Recurrent UTI\u2019s<\/li>\n  <li>Other options of Urinary diversion may be required like Vesicostomy or Ureterostomy<\/li>\n<\/ul>\n\n<div style=\"max-width:500px\"> <p><video style=\" background:#000\" width=\"100%\" height=\"300\" controls>\n    <source src=\"https:\/\/www.paedsurgeryindelhi.com\/blog\/wp-content\/themes\/twentynineteenchild\/images\/puv-video.mp4\" type=\"video\/mp4\"><\/p>\n  <\/video>\n<\/div>\n\n<h2>Follow-up and Long-term Care<\/h2>\n\n<ul class=\"list01\">\n  <li>Regular follow-up is essential to monitor kidney function and bladder health.<\/li>\n  <li>Urodynamic studies may be required in children with persistent voiding issues.<\/li>\n  <li>In some cases, children may need ongoing medications for bladder dysfunction or support for chronic kidney disease.<\/li>\n<\/ul>\n\n<h2>Outlook<\/h2>\n\n<p>With early diagnosis and appropriate treatment, many children with PUV can lead healthy lives. However, some may face long-term kidney or bladder issues.
